Woo.. Today from 1.15pm onwards i was eating till 530pm.. :)
Had company team building at Fullerton Hotel with lunch buffet, offside meeting and followed by Hi-Tea..The buffet not too bad, the salmon.. goodness... the Hi-Tea is good.. Quite relaxing.
But, after so long never went into office, the word from one of my colleague was "Are you pregnant again" when she is looking at my tummy. Oh My goodness, i'm that fat huh... When on my way home, in the car, i looked at myself... yes, i'm that fat.. time to do something to that.. i skipped tonight's dinner as i'm still too full. how to slim down??
In the past, when i think myself is fat, i will use the stairs in office instead the lift, i will walk more during lunch time.. when go ladies, i will walked up or down to 2 levels up.. and have fruits as snacks.. now working from home.. quite hard, i just need 10 steps to the toliet and 10 steps to the kitchen for lunch. at night, by the time janelle sleep, i want to relax myself, either go to sleep or just sit there and watch my HK series.. alamak. i'm just too lazy.. haiz

Teething again
Woo... Yesterday was bad and it was all because of Janelle teething again. She was very cranky n keep crying. Her night sleep n naps were distrubed. Seizures starts to breakthru. And these had made the whole family stressed up, daddy nerves snapped, in laws nagged. And mummy breakdown. I cried for hrs and now I'm fine.
Continuing my days as a mom, as a wife, as a daughter in law.

Nitrazepiam withdrawal
last week nights after dropped the night dose of nitrazepiam, Janelle nights was distrubed. She keep jerking from sleep, i stayed up in the nights to pat her to sleep. It got better after a few days... I have no idea that it is because of drug withdrawal. Just tots is her bad nights.
This week, we dropped her morning dose, it was very bad. Janelle cant get into sleep day and night. She sleep for 10 min and wake up from jerking, sometimes will screamed and cried. (just like having nightmare). 24 hours a day, she can only catch like 3 to 4 hrs of sleep in total. Am very worried.
Brought her to GP and KKH, Both GP and KKH Doctors conclued that the jerking from sleep are drugs withdrawal of Nitrazpiem. This is because nitrazpiem is a drug that will subpress the brainwave, include both normal and adnormal. and this also impacted her learning.
Yesterday night, i took a sarong to tie her up like baby time. and keep encouraging her to fight the war with the adnormal brainwave. And yes, our strong baby girl did it... although there was still jerking for the first few hours... after that she was ok and sleep thru out night, for 14 hrs.. she did woke up at 6am for milk.. our strong girl.. janelle did mummy proud again..
Hopefully this will come to the end of drug withdrawal effects.

GP referred us to KKH for H1N1 test for her flu reason being
1. She is high rish taker

Rush rush rush
Today I woke up late n late for work as going into office, so rush into office. At abt 12, my maid called me and asked me to come home as she scare because Janelle is scared. Jx can't sleep and keep waking up from her sleep.
She had seizure again. Haiz. Mummy tot she is well controlled now. At least for the past 3 weeks.
Till now, 9pm, 12 hours, Janelle is still awake with tired eyes but refused to sleep. Once she fallen awake she will wake up from a fright immediately. Very worried for her.

Yesterday was the 2nd appt with ttsh dr.
Nothing much, she just told me to nitrazepiem totally and come back in a month time. And these 2 sentences cost me $100.

Sleep Training
After 2 weeks of seizure free, i decided to give janelle sleep training.
I have 90% of these 11 months nights are awake because of Janelle decided not to sleep in the midnight. She will wake up at 2 and sleep at 5/6am. I wanted to kiss goodbye of this and return to normal lifestyle. So decided to give janelle sleep training.
I start off by delaying her night sleep from 9 to 10pm, but she got more and more excited and fall asleep at 12am. She woke up at 2am as usual, but after milk she fall straight asleep. I was very happy then, but when i was about to turn around and sleep. Janelle woke up. she played till 3am, and started to have fits. :( I ended up patting her to sleep from 3am to 6am, in between these time, she keep waking up like from shock where suddenly open her eyes big and a big movement from her hands. i hugged her to sleep, and she went down to deep sleep at 6am.
I got scolded from kogan because of this. :( very upset. am i doing wrong? where is my mommy instinct?? what should i do to correct janelle lifestyle??

Friend time
My mil call it friend time. This week my neighbour brought her grand daughter over n play with Janelle. Wa, Janelle really accompany the accompany of her friend divi, 2 mths younger than Janelle who can crawl n stand with support. Both babies talked to each other, played together, staring at each other. Very cute.

New toy
Mommy wanted to blog this but was too busy to do it. Backdate a bit. Last week, when I went into office, took the opportunity n got Janelle a play gym. I have wanted to get it for Janelle long ago but think she alr so big n had out grow play gym. Furthermore I have a set of playgym which she didn't really like it. hasitating... In the end I bought it. Cos till mow she has not reach out for things.
After a week, I saw improvement in her, she can move her hands towards toys alr. But not to the fine motor yet. At least something. :)

Great weekend ends every fast
2nd opinion at TTSH
Yesterday we went for 2nd opinion at TTSH.
The first thing she asked was family history. She mentioned that she did see many cases similar with Janelle n mostly are with family history and had outgrow by the age of 2. Kind of comfort when I heard that. She supsend that we have some family history with fit unknown.
She checked thru janelle make sure there is no birth mark on her. Then assessment her on how well she can sit.
Next she revised the medicination dosage and want us to stop nitrazepem which kogan has the most concern with. And she wants to see us in a week time.
This is a.fast n very confident review.
Hope she has a better hand on Janelle.

Summary of Janelle's Medicial History
Our daughter Janelle Gan Jing Xuan was born on 16 August 2008 in Thomson Medical Centre through natural delivery with the help of vacuum suction. She is now coming to 11 months old. She is a full term baby delivered in week 37 at the weight of 2.7 kg. Both of us (the father and mother) are not blood related.
She developed mild jaundice on the 3 days after delivery but had subsequently recovered after a few weeks. The only treatment she had was sun tanning in the early morning and late afternoon for 5 to 10 minutes after discharging from the hospital.
When she was 5 days old, she took a liver profile test Bilirubin, (Neonatal) and the result was 12 mg/dL. She also had a metabolic screening. There were no abnormality detected for both Amino acid and Acyl-Carnitine profile. We also follow closely for all her vaccination and child development assessment with our family general practitioner around our neighborhood. In general, all her growth development had been normal. She had her last immunization Prevenar Pneumococcal on 27 Mar 2009.
3 days later on 30 Mar 2009 (7.5 mths old), she had her first seizure at home when she was about to take a bath at around 1pm. According to her grandmother, who took care of her during the day mentioned that she had been all the while fine in the morning. Her eyes up rolled, both her hands and legs became rigid and the body curl slightly. Her face was expressionless. There was no noticeable jerking of the body. The seizure lasted for about 10 – 20 seconds. She quickly recovered without any cries or showed signs of discomfort. Her grandmother carried on giving her a short bath and the next episode took place when she was dressing up. It was also a short seizure for about 10 seconds.
At around 4pm, she was sound asleep in the sarong and all of a sudden there was loud high pitches scream from our neighbor’s kid (downstairs). She went into a seizure. The hands and legs were firmed. Her eyes rolled up. Her head was slight tilted upwards and her body was rigid. It was similar to the early afternoon’s fits. There was no noticeable jerking of her limps and body. This time the seizure lasted slight longer which was closed to a minute. The moment she recovered, she bust into tears and seems to be in great discomfort.
We admitted her to Thomson Medical Centre where we were then referred to KKH. On our way to KKH at around 8pm she had another episode of seizure (4th) and this time was slightly longer for about 3mins. We dashed her to a nearby clinic along the road. She was immediately injected with rectal diazepam where in a short moment she recovered and also followed with loud cries for a long period.
On the same day, at around 10pm during KKH admission she had another short seizure which lasted for 10 seconds. She did inpatient CT Scan, blood test and urine test. All results were normal, the root caused was unknown, and with no further seizure for the next two days, she was discharged on 1 April 2009 with a follow up appointment with Dr.C at the neurology clinic a month later on 30 April 2009.
The next day, on 2 April 2009 late morning, Janelle had a first seizure after the discharge from KKH. It was also similar to the initial seizures described previously. We admitted her to KKH at once. She had another round of urine test, blood test for metabolism, lumbar puncture, EEG (1 hour – with awake and asleep stage, but no seizure was captured), MRI and video EEG. In the video EEG, 3 episodes of seizure were captured. Dr mentioned that all results were normal except for video EEG. It showed abnormal electronic brain ware at the back of her head. She was later diagnosed with infantile epilepsy and was prescript with oral medication Phenobarbitone. Nitrazepam was added on a week later as Phenobarbitone was unable control Janelle’s seizure. Furthermore Janelle’s blood test presented high reading for Phenobarbitone. It was at the maximum drug level.
We stayed in hospital for about 2 weeks for monitoring the effect of drug intake. After a week on both Phenobarbitone and Nitrazepam, Janelle was still having seizure. Carbamazepine was then introduced to replace Phenobarbitone. After the changed of drug, there was improvement on her seizure frequency, and after 1 day of seizure free, we were discharged from KKH. We were given a medication plan of increasing Carbamazepine and decreasing Phenobarbitone. The medical advise was to visit the hospital should there be a prolong seizure of more than 10 minutes.
On 22nd April, she was taking 6ml of Phenobarbitone 2x (morning and night) + 1ml of Carbamazepine 2x (morning and night) + ½ tablet Nitrazepam (nightly). Janelle was still having seizures every 3 days with a cluster of 8 - 10 episodes. Drug dosage was adjusted weekly. She is now currently taking 5ml (morning), 4ml (afternoon), 5ml (night) of Carbamazepine + ½ tablet Nitrazepam (morning and night). The frequency of seizure reduced to one to two episodes every 3 to 4 days.
On 25th May 2009, during a follow up appointment with DrC, he requested to have a blood test for Lactate on Janelle, as one of the inpatient blood test results showed abnormal. However, the result taken that day was tested normal.
Both of us are aware of the long terms used of AED drugs and are very concern with her future development both physical and intellectual. We have observed slow development in her gross motor in terms of crawling, balancing her movements and speech. When Janelle was 9 months old, Dr referred us to rehab center at KKH for Occupational therapy (monthly), Physiotherapy (monthly) and speech therapy (half yearly). She is now 10 months and 2 weeks old. She is able to sit without support for only a few minutes and yet to reach out for things.

Leo baby
July 23-Aug 22
The Leo baby is bright, sunny, strong, friendly and loves attention and affection but likely to set foot where angles fear. They seek attention if positive attention is not forth coming and turn to negative behavior to get needed attention. They want moms and dads to be proud of them and want you to acknowledge their achievements and tell them that they did well. They need encouragement for creativity and remember, leo baby demands goodnight hugs and kisses from the whole family at bedtimes!
Physiotherapy - 1
Today we had our first physiotherapist session. Ms Ong noticed that from the last meet up with janelle, she had improvement.
According to Occupational therapist's notes, janelle tends to push herself backwards. In today's session, we have noticed that these had reduced.. So now she is able to sit very well. :)
today session lasted 20 min, as usual, janelle didnt co-operate, she was crying... so daddy bring janelle walk walk while mummy listening to therapist on what exercise to help janelle to improve.
this week homework:
1. put her more on kneeing position
2. put her more on side way and allow her her to support herself
3. encourage her to play with toys and also her legs.

milestone - Sit without support
Now janelle (10 months and 2 weeks), can sit without support for very long :) not so much of pushing herself backwards already. but still dont know how to push herself to the sitting position.
finally she had reached this milestone, though is 3 months late but better than never..
